The City Health Office (CHO) on Thursday announced that starting Thursday, January 2, health card issuance will take place at the NCCC Mall Victoria Plaza.

According to the City Health Office’s Facebook Page, the issuance will end on June 30. Except on holidays, it is open Monday through Friday from 8 a.m. to 5 p.m.

Applicants for the health cards will need a copy of the Application Form that can be secured from the CHO, an original Community Tax Certificate of the same year with an Official Receipt that can be secured from the City Treasurer’s Office (CTO), and Laboratory results that can be obtained in any laboratory in Davao City.

Parental consent is needed for applicants that are 18 years old and below, and Prenatal checkup/record, and Sputum Acid-Fast Bacilli for Tuberculosis for pregnant applicants.

For security guards, additional requirements include a Drug Test and Neuropsychiatric Test.

For the issuance of pink cards, additional requirements include a Hepatitis B Antibody test.

Applicants for the yellow card must also go through a basic food handling orientation. The in-charge Sanitary Inspector will release their yellow cards.
